Please read carefully and submit any missing forms.

Update posted November 4, 2008

1) Dental forms for grades K, 3rd. and 7th, were to be returned to the school nurse in September '08.

2) Physicals for K, 6th. and 11th were due to be returned in September '08.

3) All 7th graders in 2009 are required to have a Tdap immunization ( tetanus- diphtheria and pertussis).

4) A second Varicella (chickenpox) vaccine will be required by the state for all students before returning to school in September of 2009.

Students may be excluded from school if not in compliance with the state requirements.
